  1. Pick Category and choose the forms group you need. Hit Submit. All forms in that group will show up. (You do not need to search by terms, unless you know the name of the form you need.)

  2. Before you start filling the form electronically, please download it first on your computer. The "Download" icon (upper right) is the icon with the arrow pointing to a horizontal line:  .

  3. In some cases forms may download automatically to your computer upon opening.
